AutumnBridge Hospice understands that making the decision to end curative treatment for a chronic or terminal illness can be difficult. To better serve patients and families facing such decisions, we now offer the services of our transition team free of charge.

Our transition team can guide you and your loved ones in making the difficult decisions related to end-of-life care. Our team will answer your questions and connect you to resources that meet your individual needs.

When you are ready, we can help you transition from your current curative treatment plan to a palliative treatment plan that focuses on relieving symptoms and keeping you comfortable.

Our transition team includes:

• Medical social workers
• Hospice nurses
• Psychiatric nurses
• Spiritual coordinators
• Volunteers
